Palliative Care at the comfort of your home
Care Assistants and HCAs (Health Care Assistants) are dedicated to offering comforting, palliative, and end-of-life care for your loved one. Additionally, they are here to provide support to you and your family during this challenging period.
Palliative care, often referred to as “supportive care,” encompasses treatment, care, and support for individuals with life-limiting illnesses, as well as their families and friends. The primary goal is to enhance your quality of life during challenging times.
Palliative care can be provided for any length of time as a persons needs change towards the end of their life.
End-of-life care, a crucial component of palliative care, is dedicated to alleviating physical symptoms, addressing discomfort, and offering comprehensive emotional, spiritual, and psychological support during the time leading up to the end of an individual’s life.
After undergoing a thorough care assessment, a personalised plan for live-in care will be created, placing a primary focus on addressing your unique needs, wishes, and preferences. This tailor-made plan ensures that your carefully selected professional Care Assistants and HCA will reside in your home, providing the necessary support and care in alignment with your specific requirements.
The End of Life Care service offer a alternative for individuals who prefer to spend their final moments in familiar surroundings rather than relocating. Your preferences concerning end-of-life care are incorporated into the initial assessment of your needs. Detailed care plans document specific wishes and requirements. Staff undergo specialised training in end-of-life care to guarantee optimal support for individuals during the final stages in the comfort of your home.
Valuing quality care for loved ones is invaluable, but addressing the practicality of pricing is a necessity. At Care Delivered, we take pride in delivering personalised, top-quality care at a fair cost. Our approach focuses on tailoring care programs to individual needs, avoiding a one-size-fits-all approach. Each quote is unique, ensuring you only pay for the specific services you require. For a detailed discussion about your needs and to explore our offerings further, we invite you to connect with us.
Numerous clients benefit from local authority support through direct payments. To initiate the process with your local council, schedule a care needs assessment. Subsequently, your loved one’s liquid assets (excluding their property) will undergo evaluation. In England, eligibility for government support is determined if their assets are less than £23,250.
For urgent needs, we can arrange care within 24-48 hours, although typically more notice is preferable. Providing our team with ample time enhances the likelihood of finding a care assistant who is an excellent match for your needs. In instances requiring swift care, we may initially select a caregiver based on experience and then invest additional time to secure a more suitable, long-term match in the future.
Our guarantee is that if your loved one has a specific condition, they will be paired with a care assistant with experienced in providing care for individuals with this condition. Additionally, we ensure a thorough process by meeting all our carers face-to-face, conducting comprehensive background checks, and obtaining references from their work history. This commitment is aimed at providing the best possible care for your loved one.
Speaking English is a mandatory requirement for all our Care assistants. In the case of non-British nationals, we meticulously ensure that their qualifications meet the same rigorous standards upheld in the UK. This commitment ensures effective communication and maintains the high standards of care we prioritise.
Recognising the significance of continuity, especially for individuals with conditions like dementia, we acknowledge that our Care assistants, being human, require time off. To address this, we strive to maintain a consistent group of Care assistants who can step in and provide coverage when your regular carer is unavailable. This approach aims to minimise disruptions and ensure a seamless transition of care.
Our dedicated Care Assistants are available to provide assistance, typically with around 12 hours per day, ensuring your loved one’s safety and well-being. For added peace of mind, various aids are in place to alert the carer to potential issues. In cases where falls are a concern, a pressure mat near the bed can trigger an alarm if the person gets up at night. Additionally, alarms near doors prevent wandering without the carer’s awareness, enhancing overall safety measures.
